It took me all day to get this configured. I combined several ways of doing it from multiple sites.

The above instructions alone did not work for me. I also did the following:
1. Under Router, WAN Setup: Host name and Domain name are blank. Primary DNS: 64.118.82.166, Secondary DNS: 216.120.225.19
2. Under Voice, System: Provision Enable = no. Update Enable = no. RTP-Start-Loopback Codec = G711u.
